Honestly? I don't think Tsuna is ever going to turn into a mafia boss. It won't happen. I do think he'll eventually turn into the patriarch of a diminished but respectable clan with nothing but perfectly legitimate businesses, though. But never a mafia boss. And if it were the real world, Tsuna would probably have been accosted by a black car on the way to school and shot full of lead sometime around the first 60 manga chapters. :P
Well, yeah, it's weird that he wasn't wrecked by guilt about Byakuran (though I wouldn't have been either in his place) but any angst about it would have bogged the already neverending arc down immensely (it never eeeends!!), and if such considerations had been moved to the start of the Simon arc, it would have taken space from all those new characters which needed developing.
Actually, now that I think about it, I can see both pros and cons to having Tsuna angst over his first kill during the Simon arc: it would explain why he clung to Enma so readily if he didn't feel at ease to talk it out with his family (since they'd probably vehemently defend his actions), but at the same time, if Enma still betrayed him afterwards it would be much less of a gray thing... we'd be a lot more likely to hate him regardless of all the shit which drove him to it, because we all love Tsuna.
And I guess, since Past Byakuran is still alive, it was probably easier for Tsuna to brush it off his mind. "I killed him in the future... but then we changed the past and so it's like it never happened! And without the Mare rings it'll never happen either! *sigh of relief*" It's not the right way to cope, but hey... he's just a kid. It's not strange that he'd look at it from a positive angle and, once his issues were appeased, simply not think much of it again. Maybe in a few years, he'll look back and think how fucked up everything was... but I don't think he's got that much self-awareness right now.
Well, it's okay to think of characters as if they were real people, but you can't forget they live in a world different from our own. The fanwriters write some amazing stuff when they take the real-life ramifications into account, but despite everything, KHR is still a very optimistic place, and sometimes I miss that sense of optimism in fanworks.
